neck: 5-piece Maple with 4 Purpleheart pinstripes, 34 long scale
fingerboard: Ebony, unlined fretless  
pickups: MXY56
controls: volume, pan, filter, mono output
tailpiece: bird
birthday: December 20, 1995
Originally made for: Zone Music, CA
 
There's a note on the work order that this was to be completed by Christmas, so the original owner probably was having a Merry Christmas.
