It's a Switchcraft L113 jack. The 113 isn't too hard to find - but you need the one with the L so the bushing is long enough to come through the front.

We also have an improved jack available, but you sometimes have to carve a little more room inside the cavity and reshield that area since it's physically much larger than the original connector. This jack is more expensive, and requires a small amount of rewiring.
