Replacement 1/4 output jack for Series bass

Hello everybody!
Yesterday I discovered that 1/4 inch output jack on my '77 SSB is broken. I remember reading somewhere here that the manufacturer is Switchcraft.

Does anyone know the exact part number for the replacement? Any info will be greatly appreciated!

I think you can order the jack directly from Alembic.  But if it's more convenient to purchase it locally, the folks in the office can tell you the part number.

I guess, ordering from Alembic wouldn't be the fastest way of obtaining a replacement in my case because I live on the opposite side of the globe:) Of course I'll do that if there are no other options.
It looks like jacks used in Series instruments are custom made for Alembic. There are no stereo jacks with 9 terminals in Switchcraft catalogue. However, I've seen such jacks online under different brand names like Philmore and Calrad. I assume I need a stereo jack with two isolated SPDT switches? Please correct me if I'm wrong.

2 isolated form "C" contact (break before make) SPDT switches are required. You might be able to find an old National Teltroncis 335ENC or Connect Tech YHJ-800. There are Philmore and Calrad parts that have been used in the past.

The stereo jacks haven't ever been custom made. They are found as headphone jacks in consumer stereo receivers.
