I guess the buyer (at $4750) didn't pay as the guitar is back up on the bay.  I thought the selling price seemed a little high, especially with the headstock repair.

If it is just a case of someone not paying, why would you lower the starting bid by $900?
 
Notice the starting bid is now $3850.

This guitar has a really nice set of features. I'm curious as to exactly when the repair was performed, as well as if the surgery would be performed in the same manner nowadays.

Do you suppose the middle neck laminate is cherry or mahogany?

There's nothing in the build record on the neck except the Walnut portion. The lighter colored wood in the neck is probably either Maple or Birch. The middle laminate I'm not sure about, Mahogany, Cherry, or Walnut all sound like possibilities.  
 
We did the neck repair in 1980. We fix 'em the same way now.  
 
I also found this little note about a modification to the pickup selector switch that we supplied instructions for:

I don't think we did the work, but the documentation suggests there is no standby position. Instead it's:
1. both pickups mono
2. bass pickup to both channels
3. each pickup to its own channel (normal!)
4. treble pickup to both channels
 
There's no note that my dad did the work on the rewiring or if the rewiring was ever done.
 
Looks like a sweet guitar.
