Side mounted output jack needed

Hi all,  
 
Got another question, and hopefully it's an easy one.  
 
I've got a set of Alembic Activators (Jazz sized) and the matching electronics (Vol/Pan/Filter) that I want to install into a bass with a side mounted jack (Hamer CruiseBass w/ 2Tek for those interested).  
 
A direct retrofit would be ideal, but I don't know if it exists.  
 
I'm at work now, but I'll upload the pics this weekend.
 
So, can anyone point me in the right direction?

Hi Joe,
 
I'm also same situation for my Hamer B12-S.
The bass has cylindrical stereo phone jack on body side.
Almost powered guitars and basses are switched as short circuit (ring to earth).
But Alembic instruments use the switch jack.
I think following conversion.  
It's all right?

A Switchcraft 152B jack should work.  You can view and purchase one here.  In this configuration you are actually switching the negative side of the battery when a cord is plugged in.

This is awesome. Thanks guys, exactly what I was hoping for.  
 
I might hit you guys up for some further info on actually wiring this up. I'm not so good with proper wiring diagrams.  
 
Time to do some ordering!
