Checked on this back in 2009 but I don't believe you had much info on it.
Is there a chance that anything new has surfaced?
Previous owners,where it was first sold to, etc.
It appears to be all Koa.

top: Walnut  
body: Koa  
back: Walnut  
neck: Walnut (no other wood mentioned, but that's not always true)  
peghead veneers: Walnut  
originally made for: L.D.Heater Music Co.  
 
Sorry, the birthday wasn't recorded. It was returned to us by LDH in February 1977 to refinish.  
 
Not much info, but more than before :-)

I think this is in error though as the back and top and peghead veneer are Koa
(see images posted in the showcase)

Well, it's impossible to tell from photos, and Koa and Walnut are both woods that get lighter in color over time. I would wager Walnut on the front peghead veneer though, looks awfully similar to the Walnut top laminate, and I'm pretty good at wood ID. Add a photo of the back veneer and we can see if it's more similar to the back laminate or the top laminate.  
 
Now we can settle the mystery of the Walnut neck, since it's got a light wood too, I'd guess it either Maple or Birch, but hard to tell from these photos.
