Title: How to wire jack for Alembic Activators SSS set

I found a set of Strat Alembic Activators that had a broken diode, missing volume pot, and jack. I replaced the diode, found a pot that should work for testing, but I have no idea how to wire the jack. I'd be willing to hot wire the power  (always on) temporarily just so I could test it. Thanks

Two views of the standard wiring. One shows what to do with the bass and treble circuit, but since the lighting is different it may be useful, just don't use the capacitor shown.

The jack is a Switchcraft L113Volume pot is 10K audio taper
