I'm guessing a power coupling (http://memory-alpha.org/en/wiki/Power_coupling), and given the available information, I would choose a port power coupling over a starboard one, just in case your band comes under attack. Hope that helps.

It may be logical to take a Triquarter (http://www.randomsector.com/databank/library/technology/triquarter.asp) reading first.

I've always thought the spelling of that device to be tricorder (http://memory-alpha.org/en/wiki/Tricorder); a useful tool for testing the wiring at some of the more suspect bar stages.

There's one on ebay right now. You might also be able to build one. I've built a few and they are fairly simple (although the usual caveats involving AC current apply).

I would love to have the schematic for a DS5. Finding one used can be difficult and I could really use one so I don't have to bring my full rack to rehearsal.

I think I can locate all the rest, but the 24 to 30 VAC center tapped transformer, I just don't really know much about.
Can anyone suggest a specific part that would work (preferrably with a location to buy it online)?

Well I won't have to send any out. Look for a Stancor TP2 on the transformer, dad says these are usually quite easy to find surplus.

Any issue with connecting a Jensen transformer on the output jacks?

Can anyone here who built one of these give an approximation of how much it cost to build?
Or better yet, does anyone have one of the old blue boxes they have maybe replaced with a rackmount they want to sell?

I just built one in OZ cost me about $55 AUD the diecast box was the dearest part at $35

You can use the above schematic and build it that way. I got the single rack space 19 boxes from Markertek. I included some other stuff (front & back outputs, mono/stereo switching, effects loops, etc.), but this will get you going.

FWIW, I had my amp tech in London make me one and incorporate it along with an Fostex 4 channel mixer into a 1u rack mounted chassis. So I had 2 channels for the Alembic and two other channels for mono basses with a stereo effects loop. Works a treat!

with ground lift as well ?
